		<title>Comment on Back Pain &#8211; SI Joint Dysfunction by Jed Downs, MD</title>

		<description>I came across this blog while searching for outcomes data on S-I fusions.  I practice osteopathic manipulative medicine and have observations and opinions developed over the years.  First not all physical therapists will recognize S-I dysfunction.  those using a MacKenzie approach , unless things have changed, have denied the movement of the S-I joint occurs.  If the S-I joint is the source of pain that is not a good starting point.  Most American Osteopaths will recognize a number of different patterns of S-I dysfunction, these include posterior and anterior innominate rotations often coupled with either an anterior or posterior sacral nutation, anterior and posterior sacral torsions, innominate upslips and downslips and then pelvic pain can also come form pubic dysfunctions.  Complicating this is the fact that these mechanical problems can exist in combination with each other.  
The next level of complicatoin is that sometimes the mechanical problems of the pelvis can be secondary to some other problem.  The most common problems will be visceral restrictions.  The ones I find most common clinically include restrictions pof the ilio vesical ligaments and the broad ligaments of the uterus.  Other sources can include restrictions of the iliac arteries and the cecum and the sigmoid colon.  Most commonly the visceral restrictions are secondary to post operative scarring but they can also occur as a result of trauma, for instance when the pelvis is relatively fixed during the rapid acceleration and deceleration which occurs during motor vehicle accidents with seat belts in use.
Not recognizing the correct pattern of restriction will lead to application of inappropriate and ineffective treatment techniques and subsequent treatment failure.  Before moving up the interventional treatment chain, proper evaluation and treatment ideally should be pursued.
There are unstable SI joints, especially after  falls on the buttocks, some patients I have managed have responded reasonably well to prolotherapy.  I have yet to meet a patient who is happy that they underwent an S-I fusion.  An unfortunate problem is that most allopathic physicians (M.D.&#039;s) have not been taught to recognize Si problems on physical exam.  Chiropractors should be able to recognize S-I joint problems, but I am not well versed in their assessment techniques.  I do see treatment failures coming from the  chiropractic community.  I, too, have treatment failures.  There is a need to move up the interventional chain for some.  None of the above discussion deals with the interaction between the spine and sacrum or the cranium and sacrum via dural connections, but that is a discussion for another diatribe.</description>

Massage is good for a number of conditions and illnesses, and generally more often than not it is the feel good factor and relaxation that are the positive effects post massage, rather than any medical cures. However we must remember that it is also contraindicated for other illnesses such as Cancer, as massage could encourage the cancer to spread and cause possible illnesses such as lymphedema.
Overall though for the general population massage should be a regular part of most peoples lives both for general feel good factor and the prevention of injuries.</description>
